Rectangle Health, a commercial entity based in Valhalla, NY, experienced a data breach affecting 2,095 individuals, including 11 residents of Maine. The breach was discovered on September 5, 2025, and involved unauthorized access to personal information. The compromised data included names, dates of birth, and Social Security numbers.
What Happened
Rectangle Health was recently informed by Salesforce, the provider of a software platform used by Rectangle Health to store customer information, that the platform was subject to unauthorized access on August 14, 2025. Upon learning of this, Rectangle Health immediately launched an investigation to determine what data may have been impacted. On September 5, 2025, Rectangle Health finalized this review and determined that information related to certain individuals could have been impacted. To date, we have seen no evidence of any fraudulent use of any data as a result of this event.
The breach occurred on August 14, 2025, due to unauthorized access to Rectangle Health’s platform. The affected individuals potentially had their names, dates of birth, and Social Security numbers compromised.

Affected users should remain vigilant for incidents of fraud and identity theft by reviewing account statements and monitoring free credit reports. They are advised to report any suspected incidents of identity theft or fraud to their credit card company and/or bank.
